AAC Form and Academic Advisement



The Application for Admission to Candidacy (AAC) form is part of required paperwork for graduation. IPT master's degree-seeking students are encouraged to use their AAC form as a planning and communication tool with their academic advisor.

IPT Master's Degree Students

Students pursuing an IPT master's degree should create their AAC form and start entering information into it as early as possible, and no later than completing their 19th credit.

While completing the AAC form, students should discuss their plans with their academic advisor and use the recommended course paths and 3-year course plan documents as resources.

The AAC form helps students:

  • map out the course path to graduation
  • better communicate with their academic advisor
  • determine and prepare for their culminating activity (thesis, project, portfolio, or comprehensive exam)
  • graduate within their planned timeframe

Students can revise their AAC form anytime when necessary, until they submit the final version of their AAC form to the Graduate College as part of graduation paperwork.

HPT Graduate Certificate Students

Students pursuing a graduate certificate in HPT will submit their AAC form as part of their graduation paperwork by the indicated deadlines.
